Teaching Students w/ASD 3.2: Examining Student Files

Where do I look for what?

Student files at my school can come in one, two, or three different colours. Every student has a white file, but students with a designation also have a red file containing information on their designation and past and present IEPs among other documents related to their designation. Students who are English language learners have green files containing their progress reports and access to resource time. Below you will find in detail what kind of documents you will find in each file to help you navigate student files more efficiently.

 

White File- every student has one

  • birth certificate
  • school application
  • kindergarten transition information
  • student information verification (a form used to verify student’s caregivers and address every year)
  • past report cards including core competency self-assessment and records of ongoing communication of learning
  • consent to send commercial electronic messages

Red file -students with ministry designations have one

  • Archived IEPs
  • Current IEP
  • Referral for designation
  • Archived assessment and reports (SLP reports etc.)
  • Record of communication: 

-letters to parents

-SBT meeting minutes

-contact sheets

-record of communication sheet

-medical reports

  • Copy of diagnosis by specialist
  • Copy of diagnosis by BCAAN (if diagnosed with ASD)
  • Agency Involvement documents such as:

-private behavioural consultation

-POPARD

-Vancouver Coast Health

-Canucks Autism Network

-Sunnyhill Hospital report

-SET BC technology review

-Ministry of Children and Family Affairs

  • Evidence that designation effects academic achievement and supports to achieve success such as: 

-inclusion consultant report

-instructional support planning process

-POPARD consultation

-School-based resource support

-SSA (EA) timetable

-augmentative and alternative communication SLP consultation

-attendance

-accommodation list

-social skills checklist

-sensory needs

-request for SLP

Green file-English Language Learners

-resource timetable

-language development support strategies

-annual ELL instruction plan

-ELL Reports 

-Unassisted writes

-written language checklist
