Teaching Students w/ASD 3.4: Wraparound and Case Managment

In my experience students with ASD don't receive and integrated program of support. I shared my frustration about this in a previous post about and can see many colleagues that have posted share this frustration. 
In my case, not enough collaboration or communication between SBT members, SSAs(EAs), and outside agencies coupled with the inability of caregivers to communicate proficiently in English made for a very fragmented "support" program for one student of mine over the last two years. 
How could this be improved?
  • teachers or specialists leaving (both within and outside a school) should be required to brief new staff taking over student's case
  • new teachers or specialists replacing someone should be required to make contact with everyone supporting student
  • an updated list of everyone working on students case should be kept in the student's file
  • specialists could provide caregivers with business cards or note at the beginning of a new school year with the intention of passing them on to a student's new teacher
  • the teacher could provide a survey at the beginning of the  year asking about all of the outside services a student receives and names of contacts
  • SSAs(EAs) could communicate better amongst each other and brief new SSAs taking over students they support

It would be great if there were just one platform that all specialists and teachers on a student's case had access to, to upload notes, strategies, and most up-to-date information. I suppose there would be too many privacy concerns.
